This is the CDC version of the International Certificate of Vaccination (ICV). It is approved by the World Health Organization (WHO) and issued by U.S. Department of Health and Human Services. Although it is known popularly as the “Carte Jaune” or “Yellow Card”, the color may vary from beige to light yellow. All booklets are authentic, not reproduced. Printed by the US Government and authored by the CDC It is not designed for COVID-19 vaccination. A special Covid-19 vaccine card, published by the CDC, will be issued to you by your vaccine provider after you have received the vaccine . The Certificate is used for recording routine and travel vaccinations and medications to prevent disease (prophylaxis). It provides official proof of Yellow Fever Vaccination for International Travel. Additional pages have space for your health history, regular medications, and allergies. The Yellow Card is recognized internationally and may be required for entry to certain countries where there are increased health risks for travelers. Before departing for a foreign destination, a traveler must visit a healthcare provider and receive the required vaccinations, which the doctor must list on the form per the instructions and sign and date. The Yellow Card must then be kept with the traveler's passport as he or she travels, since it serves as a medical "passport" of sorts as proof of vaccination, including booster shots. An essential travel document for some areas of the world
There is not much to say about a standard government form/document. It is authentic and if you need it, get it. Your doctor will either be impressed or upset when you ask them to fill this out but you will want them to do it, or at least one of the nurses in the office, since it is an official document. I would add flue, and covid vaccinations along with the usual vaccinations.

Limited space for Vaccines
I guess we have more vaccines than when I needed it. To list them all out, you'd fill this up quick. There's a 3 line section and then this "other" that leaves you more room. You can add medical contraindications to vaccines, personal health history, regular meds, and glasses information as well. It's a thick paper, not cardstock as it was 20 years ago. Still should fit within a passport, although it comes in a nice plastic holder.

The only place I found to get just one CDC vaccination card
Filled my first shot card. Needed another one. This is the only site I found where I could buy just one shot card. Surprisingly pricey but it is the CDC's card.
