Prompt Delivery, everything you need
I have no idea how I crushed the lens cover on my phone. It was fine, then it wasn't. I found this little kit and an associated YouTube video on swapping it out. It looked easy, I ordered one.Upon delivery, I instantly started swapping the parts out. I removed the old cover and all the shards and dust being careful not to disturb the camera. I cleaned off all the old glue. When I went to install the black 'bezel' (I don't know what to call it) I had some issue separating the backer from the adhesive. Indeed, I separated the adhesive from the bezel before I refocused and made sure to separate the backing. The lens was easier.However my results were not good. It seems I left a shard of the old lens behind, something I didn't see right away. My first picture looked good, all since then look blurry. The shard is now against the back side of the lens, a location that it couldn't have been initially. I doubt this to be a problem with the product, more my deficiencies in making sure I removed all the debris. I like the product enough to order another one and try again. I hope to see it in a day or two.Addendum: The second kit showed up. After installation there is what appears to be a 'crescent moon' of something on the orifice which I assume to be misplaced adhesive. The effect is blurry images for pictures in close range, not as noticable for pictures in a longer range. In all fairness, I have not followed up with Bonafide Hardware.

Worked for me
This was a good experience. I had no issues with fixing my Samsung galaxy s5 glass lens for the back camera along with the Youtube video noted by others (and this product does not have the cover on the glass, but it does not take away from any of the effectiveness of the product as it was sealed properly for shipping). I can finally take pics with it again as well as scan QR codes. Thank you!

I ordered 2 kits just in case I would have ...
I ordered 2 kits just in case I would have problems. Sure enough I did have problems (The kit came with no instructions):Make sure your environment is free of dust and warm to 75 degrees Fahrenheit or so, because you will have problems pealing the various adhesives as happened to me, but I was lucky I ordered 2 kits.1) The adhesive for the glass lens was impossible to peel off the protective paper, so I ruined it. I used the one from the second kit, I warm it up with a hair drier before pealing the protective paper.2) On both kits, it did NOT come with a protective film over the lens as expected from the youtube installation. I tried to check if the film was there and in the process I scratched the black coating off of parts of the lens.3) The metal frame came with 2 adhesive rings, one on the bottom of the frame and another on the inside of the frame. When I tried to pull the light blue protective film, the adhesive stuck to the light blue protective film, and I could not use this frame. I used the one from the second kit, I warm it up with a hair drier before pealing the protective film.I was able to complete the repair. I hope this helps other people before they start the repair.

Thought It Was Defective; Turns Out My Phone Was The Problem!
UPDATE: This was my second time buying this lens, because I thought the first one was defective. My original camera lens broke months ago and I saw a Youtube video recommending this replacement. I did it myself, but suddenly my camera wouldn't focus! Obviously I thought it was the new lens, since I saw similar complaints in the feedback. However, I just installed my second replacement, which was STILL blurry, and decided to try some other solutions. Apparently the Galaxy S5 is prone to having hardware problems, and shaking or tapping your phone (for me I have to hit it pretty hard) will actually correct the issue and give you your focus back!! The lens was never the problem, and I didn't even have to buy it this second time. Now my pictures are clear and beautiful, even if I look ridiculous tossing my phone on the table or floor every time I want a picture.I've updated my rating because this lens itself came quickly, was easy to install, and works fine. It was just my phone the whole time.

No protective plastic
If you came here from the YouTube tutorial, be aware that there's no protective plastic on the lens like he shows on the video. I ended up botching the installation trying to find the plastic. I even removed the adhesive thinking that might be it, and could not get it to focus after that. Luckily my package came with two lenses, so the second attempt worked. I also ordered gold but got silver, but that's not a deal breaker.Update: it turns out the second attempt didn't work either. The camera only focuses at close range.

There are no instructions included, but you can easily find YouTube videos online that guide you through how to replace the lens. I ended up ordering this twice. The first one I ordered fit my phone and did the job, but I accidentally installed it with dust under the lens. This caused the camera to not focus correctly. I ordered another and took extra care to remove any dust from the lens before setting in place. Now it works perfectly, and my camera focuses with no issues. Even having had purchased two of these, it was still a lot cheaper to repair myself.

Great fix
I dropped my phone and it fell just right to break the camera glass only.Camera was uselessI saw this and figured it was worth a try, perfect fix.

A simple fix for a poor phone design
The lens was simple to replace and this was a perfect fit. I used a pair of sharp tweezers to pry off the bezel and then scrap off my broken lens and old adhesive. You just grab the tab to peel off the adhesive cover and stick the new lens in place. Then with a fingernail scratch the outer edge to remove the protective film. The only time you need to use your head is to align the notch of the bezel to the matching shape of the phone. The whole process took less than 15 minutes. Now I can take pictures of my grandbabies.
