What tog is this item please

It says 1.0 tog, it's very thin material but its tight

If baby turns to the side at times would this still work/be ok to use?

I think you shouldn't use it after the baby started turning.

My 4 weeks old is about 5 kg . what size would work ? shall i go with s or m? what was your experience pls? thank you

My little one is 12 weeks and 4.7kg and is in the small. She still has space and is nice and safe in it. I would go for a smaller size if your between sizes for safety as if the neck hole is to big it can be unsafe.
