Q: Can I use these to turn a bag into a lunch box?

You can use it to wrap the lunch box to achieve the effect of insulation

Q: Does it work with hot food?

It will, up to a certain point. The inside is a soft Styrofoam/plastic and the outside mylar. I am sure that it will melt at a high enough temperature.

Q: Do i put the ice packs inside the isolation or on top of it to keep items frozen?

I generally put them in the middle. I pack a lot of little refrigerated items, and my goal is to keep them cold, not frozen. I have not received any negative feedback from my customers yet. I don't know how well it would keep frozen items frozen. But, if your question was to put the ice packs inside or outside of the thermal bag, well, you have to put the packs inside. The whole purpose of the bag is to keep the outside temperature from getting in.
