Ok, description says 12 inches and amazon selection tab shows 16 inches. which one is it for certain? can't get it wrong. needs to be 100% accurate

Sorry I did not read your question until now. It measures 16.5 inches end to end. The 12 inch description is probably referring to the grabbable length.

I only received 3 of the 4 bolts that attach thegrab bar to the base plate. Can you tell me what size bolt to get?

Don’t know the size but they were too short so used longer ones from some I had on hand

Is the non slip diamond shaped, as those hurt hands?

I’m not really sure what you’re trying to ask. The nonslip pattern isn’t enough to hurt your hands. It’s not like sandpaper but it’s definitely better than having a non textured surface.

How are attached to a plastic surround ?

There has to something substantial as "backer" for the screws to go into. It's usually wood that's under/behind the paneling/drywall that the plastic surround is attached to. If there wasn't backer specifically installed in the desired location, you'll have to determine where the wall studs are, and get at least 2 screws, on each end of the bar, into the wood. The drywall, paneling and surround will NOT provide adequate strength for the bar. It will NOT be ADA compliant, nor will it be safe or reliable in an emergency situation, which is exactly when you need it.
