Can this be used on an 11th month old?

If the 11 month old keeps them in, it's safer than using nothing at all. They are very easy to mold to the ears, so that shouldn't be a problem. Just remember to wipe/dry off their ears when they are out of the water.

Are these small enough for an infant’s ears?

It is probably too big for infants ear, but you can pull some off of it once warmed up.

I recently had to get tubes put in my ears. I am wondering if they will create too much pressure while putting in/taking out (also hole in eardrum).

I have tubes in my ears and they have not caused any pressure in my ears. They are sticky and don't really go down the ear canal like regular ear plugs. I love them!

Can you use these and still hear music at concerts?

Affirmative. My son can still hear me and responds when we are at a noisy pool or the beach. I don't believe these earplugs are meant to be used for sound suppression as much as they are intended to keep the inner ear dry.
