Q: How should this be formatted and setup to use as a boot drive for macintosh?

Even after reformatting as an OSX drive (it comes formatted for Windows), it cannot be used as an external boot drive. However, I used Drive Genius to format it as a "Bootwell" drive, and that seemed to enable it to be used as a boot drive. You will need to clone a current drive to the Samsung T5 if you want it to boot up with all the apps you used to have.

Q: Can I use this as an external backup drive? Is there a back up software included?

Do not use the Password Protection. If you just installed, you can disable the Password Protection feature on the Settings screen of the Samsung Portable SSD software as follows: Click the "Set" button on the Main screen, switch the "Lock" button from ON to OFF, enter a password, and save it. Otherwise you cannot expect unattended backup's.
