Q: Does this include the silcone band

Yes, there are 4 bands included, as shown in the picture. However, be aware that the earplugs are not sufficient ear protection if you go to an indoor shooting range. I used them once and came away with a partially dead right ear due to the high pressure sound level of gunshots (my own 9mm and 22LR, plus the sounds of others shooting higher calibers. My right ear was ringing and attenuated for about 4 days, then recovered. I recommend these, with caution, for outdoor shooting only. For indoor ranges I recommend getting a full ear cover sound attenuating set of ear muffs.

Not sure. Not good for the gun range.

Q: Where can i get replacemt ear plugs once the extra ones are finished?

Amazon. Or just buy the whole set again. They are inexpensive.
