I'm 6'0" 325lbs, wear a 3XL shirt and this easily fits around me. I would bet 4-5xl could still got in this.

Yes, they do. I bought the 140lb vest 6 years ago. It's still holding up to being used every single week, loaded to the maximum 140lbs for the last 3 years. While the shoulder pads aren't very thick, or heavily cushioned/padded, they make a significant difference at 140lbs, simply by being wider, and distributing the load over a wider area. This vest puts all the load on your shoulders. The pads definitely make a difference.
