Q: Is the toe opening too tight to hurt the toes?

It does not hurt my toes at all. I have total relief from the pain that I used to have from swelling as a result of poor circulation from having many leg surgeries. Obviously you will need to get the proper size for your needs.

Q: According to the size chart the medium will fit my ankles and the large will fit my calves. What do I buy?

I'm not a medical person, but common sense tells me that a medium would be too tight for your calves. Also, I think more problems are in the calves, so that would make more sense to fit the problem area.

Q: Where is the measurements

You measure the calf of your legs. There are guidelines when you order to see which size fits you. I hope that helps.
