These are the CMS claim forms pre-printed so you can just print out the filled in form fields from your software. These worked just fine for my needs and were reasonably priced for the quantity.

The HCFA form is necessary for filling medical claims with the few insurance companies that have yet to join us in the 21st century. 95% of the insurance claims that I file are done electronically but paper claims through the USPS are needed for the other 5% if I want to get paid. The HCFA forms are printed on quality paper and 500 for that price is a steal. Hopefully they'll last me until all the claims are finally done electronically and I'll never have too order them again. If I do, it'll be from this seller!

It would be really hard to mess up preprinted forms. this product lives up to the hype

Very happy with these forms! Although the printed form is a little lighter than I am used to, the insurance companies don't seem to mind. I was, previously, purchasing CMS 1500 forms from the large Medical related supply vendors and the forms never printed consistantly ~ I would have to constantly adjust my printer to populate the fields correctly (always off center, too high, too low); some payers would even misread the alphanumeric fields because of the way the font would misalign in the field. With these forms, from this vendor, the print is in the same place in the field & doesn't need constant printer adjustments. Packaged well & shipped promptly AND they are 1/2 the price of what I had been paying. I couldn't be happier with the product & will definitely reorder.

This is a great value if you only need a small amount of 1500 HCFA forms. The paper is thinner than my previous order from another seller and I had to reformat my printer, but they are getting the job done.
