Q: What part of these are polyester? The description states 100% cotton, 100% polyester.

Fabric is cotton. Waist band is synthetic.

Q: Per this chart he's a 3t (3t/4t in clothing); 34 lbs, 37" tall; size 6 diaper. some say size up - do they really shrink that much?!

I think they fit my little guy perfect from the chart I didn’t think they shrunk at all

Q: Chart in description on top shows that 4T is suitable for weight 33-37.5, while another at the bottom shows 39-49. Which is true?

These are not true to size. You will need to go up at least one size and I would recommend two sizes. As soon as you wash them they shrink an unbelievable amount. My son was only able to wear them once after washing and I would never buy them again.
