Q: Does this have ice pack in arms and chest?

The ice or frozen areas lie in all the regions you see in the image. So, it wraps over the front and back of the shoulder - and extends partially down the upper arm.

Q: How should it be heated? It's advertised as both a cold & hot pack.

I have not heated mine but in the past I just put them in the microwave but start for just a minute. You don't want to get it too hot and put it on your shoulder and get burned.

Q: Why is there a woman modelling this? Is it something about size? I'm a 6-2 man

As a 5-5 woman with 2 torn rotator cuffs I don't have a problem with the image. You may have a bigger issue with the size of the upper arm ice area. I do. I did mention this to the maker and they are looking into making it larger for muscular men and large armed women.

Q: It says it can be used for up to 5 years. Does that mean you get a warranty from the seller?

Not sure about a warranty but I can say that the manufacturer stands behind the product totally. Depending on how much you use it and how tough you are one it, I can understand it lasting approx 5 years.
