Hello...What is the wiring way, does this lamp use ballast?

This is 365nm UV BL replacement bulb, it is replace for the 30W bug zapper, Its role is that the light it emits can attract insects. It can't work alone, it must be installed with the ballast, but the ballast is already installed inside the bug zapper, so in general, just replace it.

are these unfiltered? I'm looking to replace a T-8 unfiltered UV blacklight fluorescent 15 watt bulb (18in) for a silkscreening exposure unit.

why don't they last longer than 90 days

Because they are not worth buying. Honestly they were supposed to send me a replacement or give me a refund. But they never did either one I had the same problem they would burn out in 2-3 weeks.

No, this the UVA UV/BL light bulb , it is replacement for the bug zapper/ insect killer/ mosquito killer, not UV-C gerilicidal bulb.
