Q: Can you hang the paddle by the handle as well as the blade? What is the measurement between the two hooks?

You can technically hang it by the handle, but it wouldn't stay there. It would swing or wobble around. The grooves are made for the blade specifically. Mine measures 2 1/4 inches in between the hooks.

Q: Does this work for paddleboard paddles?

I believe so. I use it for kayak paddles.

Q: Can this be screwed in to drywall or does it require a stud?

With the right type of anchor it can be. Screwing it to a stud is easier and more secure. The screws that come with it are meant to be screwed into a stud.

Q: How far does this extend from the wall?

Exactly 6 1/2". It has worked perfectly. Keeps paddles hung and out of way.
