After you make your bowl, you place the holder on top of it and lay your coals out along the inside (as even as possible for heat distribution) and you'll have to do a few pulls (sucking the air/smoke from the hookah) to help the coals start your hookah properly. I suggest getting a handheld hookah pump (cost less than $15) as this will keep you from sucking down on an empty hookah. It's one of the best I've ever used.
