How wide are the grid bars appart on the 18"?

The grid cover openings are rectangular. Each is about 3/4 inch long, 1/2 inch wide.

What is the optimum distance if I want to hang it over the cage? Is there a maximum distance?

There is no maximum distance. The only downside is that the higher you go; the dimmer the light is on the bird and birds REALLY like light.

My vet recently told me I need to replace the bulbs in any cage light every 6 months due to losing effectiveness. Is that the case with this one?

While LEDs do not burn out like fluorescent lamps and other bulbs they will, however, degrade and dim over time. The diode itself will begin to emit less and less light as the years pass. Still, LED lamps can last over 25,000 hours. So the answer is that led will over time stop producing quality of light birds need over time as they get dimmer but talk to manufacture how long their model last before loosing its effectiveness.
