Is there a way to extend the inlet to the surface?

Yes l glued a 4 inch ABS coupling to the bottom of the inlet and then you can add any length of 4 inch ABS pipe you need . l used ABS but you can also use a PVC coupling and pipe just make sure you use the right glue the one l used is good for both PVC and ABS. Just as a side note after l installed mine l put a piece of heavy duty filter cloth over the top and secured it with a 6 inch stainless gear clamp to keep any debris from getting in it works like a charm , l hope this helps.

Does this item come with the filter cap? I purchased mine & it did not come with the cap?

If by the filter cap you mean the grill in the photo yes it does. When l installed mine l put a piece of black landscape fabric over the top of it with a stainless gear clamp to keep out any debris and still allow it to breathe it worked great . l hope this helps!

What are the dimentions of the nds flo-well surface inlet (fwsd69)?

the top is 6 3/8 inches from edge to edge its 9 inches long and the bottom opening is 4 inches wide

Could you cover this with landscape fabric and rubber mulch? I intend to put a dry well under my kids playground area to help with the pooling water.

Sure, just make sure you put a drain cap on it and you might want to cover the drain cap also with fabric to keep the tank from getting full of mulch.
