does the knife come out when you pull the trigger?

No it doesn't. However, the front aim on the tip of the barrel serves as a trigger.So, when you pull the front aim lever the knife springs out like a switch blade. I love the knife. I own two of them.

Is this a good knife and a decent size

I never received the item, because the send me wrong item* pink handcuffs “ which is I never ordered, and after get in contact with the seller, he do refund! So I never see the item alive in my hand, and this way I cannot answer that question!

Is it revolver sized or just a small knife?

Its more or less in between those sizes, because its not a small pocket knife, but not as big as a revolver.
