What is the maximum waist size please?

The maximum waist size is 45.75 inches ( forty five and three quarter inches) when using the existing punched holes. There is no room to punch extra holes in order to make it larger

There are two types of belts in the picture. Which is for sale?

Yes I see what you mean regarding the 4 photos they're showing for this tool belt. I purchased the belt in 2016 and Amazon has asked me to attempt to help you. My belt is photo 2, 3 & 4 I'm unsure if the first photo is of the same belt. The belt is a two tone leather,as in those listed above. I'm unsure how much I paid in 2016 but the belt I have is well worth the money I paid. It's a very strong belt with very thick leather, maybe to thick, it's better now than when I purchased it, softer! If you're after a good tool belt then I think this scores probably 7 out of 10, yes there are better ones out there but they cost much more. I can't offer any more advice but I hope this has clarified a little for you.
