Q: Can this be flush mounted as in the picture.

That's how I installed mine. I had to make a false ceiling though because a joist was in my way

I had a full remodel and an electrician did all the wiring so I don't know how to answer the question. However, I do have it set up on a 2 switch. One for the rainfall and the other is for the light above my clawfoot tub. I do love the way everything turned out. I have a a master walkin shower with 2 shower head and the rainfall.

Q: One or the silicone rubber nozzles is missing on our unit. How do i order a replacement silicone nozzle?

Mine came with a baggie with replacements.
