Can an earclip be used with this unit, and if so what setting is to be used?

Probably not since an earplug clip typically uses a stereo jack as a connector

Does this really do EMS electronic muscle stimulation, or does it just do TENS? How do you select EMS mode?

Before purchasing, I asked my PT the same thing. He indicated that EMS is more intense "shock" and may have different stimulation frequencies and patterns. The unit has such patterns. I am more interested in pain relief (TENS) and therefore do not use the EMS function. Th lower 2-rows of buttons have multiple functions based on touching the button multiple times in a row. For example, if you press the "KNEED" button multiple times it moves from a TENS mode to an EMS mode, changing the highest intensity and pattern/frequency. I did use this to create an AB plate with electrodes at the top and bottom of ABs. it does work, however I would like it to be stronger "shock" for my ABs.

Can the pads be reused or do you have to replace them after every use?

Yes! You can use them repeatedly. If they lose their stickiness, add some water and leave them face up to dry. This will reactive the grip. We used the same pads for months end before switching to new ones. The replacement pads are very inexpensive...
