Is this item already put together or does it need to be assembled?

Partially out together. You need to attach a few hinges.

I put this together but it does not fold up like it shows in the advertisement. Has this happened to anyone else?? Very disappointed if so..

Mine folds like advertised. I found the assembly instruction confusing at first and made a mistake assembling the panels. Once I figured that out, it worked as advertised. May I suggest you double check how the panels are assembled,

Is this sturdy on carpet?

Should be fine if carpet surface is even. Its got some weight to it. (Mine is on a hardwood floor.)

Is it supposed to be missing 2 hinges on the bottom of the top portion or is mine defective?

There are four hinges for the top and bottom panels (where they fold)

I will admit, I was very skeptical when I read the assembly instructions. BUT... they actually were accurate. It took about ten minutes and it looks FANTASTIC. And it folds easily and is lightweight enough to move quickly, easily, and safely. For the record, I wouldn't use this around young kids. It is VERY lightweight Even as toddlers, my kids would totally have knocked this down. For an adult household, or even one with older kids, it's fine. I followed the directions and got someone to help out. I probably could have completed the assembly myself, but the extra pair of hands not only made the job easier, but the entire thing was up and ready for use in less than ten minutes.
