Q: Do I need to insert the ps4 disc on my ps5 console every timeo play fifa21, or do I just need the ps4 disc only to download and install the game once?

Yes the disc is required to play.

Q: I have a ps5 digital. if i buy the ps4 version does it come with a ps5 digital code?

If you buy the PS4 digital version, then it comes with the PS5 version digital. What would happen is if you have ps5, you download the PS4 version and I believe it gives you an option to do the PS5 upgrade and then it will download the PS5 version. If you get the PS4 version physically however, you do get the PS5 version but you will be required to insert the disc in order to play the game. Regardless, you get the PS5 version for free with the PS4 version until the launch of FIFA 22 which is in either September or October of 2021
