Q: What is the width? One section says it's 39.37" and another says 42.9" -- Also, what is the height between the shelves?

39 3/8 " wide 8 1/4" Between shelves

Q: Can I add shelf to make higher?

It is highly recommended that you do not add or take away from the product. It has been safety for the three shelf design and should be put together exactly as per the instructions.

Q: can this be assembled without the middle shelf?

If you want the height to be the same it would not work. The posts are only the height of each shelf.

Q: Can i put a 65 inch tv on it

Nope. This stand could not accommodate our 50 inch TV and had to be returned. It will certainly not be enough for your 65 inch.
