The adjustable belt has an open area in the fabric and a zipper. You can slide your insulin pump from the side through the opening as far as your infusion set tubing allows. In addition, there is a clip inside the belt at the far end, you can secure items like a key chain. The zipper is located in the center of the belt, zip to open if you need to add an item or slide you pump along the belt. Hope I answered your questions.
