Is this the latest model , which is the latest, D or H model?

U2717D is the latest model -- the H you're probably referring to is their older model U2715H; the main differences being a new panel (a Samsung PLS -- their take on IPS -- vs LG's IPS), thin bezels on all four sides (their "Infinity Edge") vs. on 3 sides, 1 fewer HDMI and 1 fewer USB, more easy access to USB ports

Essentially the same monitor. However, Dell for some reason removed the mounting tabs for the AC511 soundbar on the newer U2717D model.If that's not an issue for you, than I would say go with the newer model. I have used these in medical offices because the color accuracy is important. Some people say the color doesn't look good on these but that is because they are used to the bleeding over bearing color settings on most monitors and TVs.

How well does this work with a 2015 Macbook 12inch?

I have it connected to a 2013 or so MacBook Pro (13") and it looks very good.
