I purchased mine based on my shirt size... i wear a medium, but I ordered up... they have adjustable straps so you can make it to fit if needed...

Does this have mesh on the back?

No mesh. It has similar padding to the front but not as thick. I used it for kayaking this weekend and it worked great and did not get in the way of my kayak seat.
