Has anyone used this sized cushion (18X18) in a power-lift recliner and found it too high or too small?

I use it in a power lift chair. It works wonderfully for me! I’m 258 lbs., 5’10” and am very pleased with it. I’m thing of getting a second one for my porch swing, it can be used for a number of uses.

how does the hand pump work?  Does it require a lot of strength and energy to use for seniors citizen with no real arm strength?

There is a valve stem that protrudes from the side of the cushion that looks like the valve stem on a car or bicycle tire.  The cushion is inflated by pumping air through this valve stem just like a car or bicycle tire is inflated through a valve stem.  When the cushion needs to be inflated, you must open the valve stem by twisting the valve open with your fingers much as you would turn a small knob or dial between your thumb and pointing finger.  The hand pump is made of a very light weight plastic material and looks like a large syringe with a barrel and plunger.  The tip of the "syringe" hand pump is of a large enough diameter to fit over the end of the cushion valve stem.  So, start by twisting the valve stem open.  Once the valve stem is twisted open, slip the tip of the syringe over the end of the valve stem.  After this is done, inflate the cushion by pushing and pulling on the plunger of the syringe pump.  After the cushion has been inflated to the desired level,  the syringe pump can be disconnected from valve.  The valve must then be twisted closed to keep it from losing pressure.  Inflating the cushion does not take a great deal of strength and energy.  The first inflation of the cushion takes the most energy because you would be starting from a flat cushion, but is not too strenuous even at that.  Maintaining the cushion at your preferred level of pressure after the first inflation takes little strength and even less energy since you will be starting from a semi-inflated cushion as opposed to a flat one.  Twisting the cushion valve stem to open it does take a little manual dexterity and may be a challenge if you have difficulty using your fingers (e.g. arthritis, etc.)  All in all, with everything being said, the cushion is easy to inflate.

Can I use the 18" wide by 16" deep cushion on a 16" wide by 18" deep seat by just rotating the cushion?

Yes you can.  The pump valve behind the zipper can be in front, side or back with no problem.
