Can you fully straighten the leg with this ? All the picutres show some knee flexion

Yes you can straighten and bend your knee. The brace is solely for support in keeping the knee for shifting and lessening pain from sudden jolting. I found it helpful with a torn meniscus.

Measure from middle or top of knee?

Please measure the leg circumference 4 inches above the knee.
