Q: Will I have a problem anchoring these in a laminated veneer joist? I had multiple lag bolts literally twist in two trying to get them tight.

You will be fine with the correct size pilot hole for the bolt diameter.

Q: How is the curved washer used? What orientation to you place it with?

Washer goes into the 1.5 inch hole, over the threaded stud, curved side down. 1/2 inch nut goes on top, mates with flat side of washer.

Q: Will this work on carpeted steps?

Not sure what you mean. This is meant to secure a Newel post which is not on the steps but at the top and /or bottom landing to support the handrails. So the carpeting on the steps should have nothing to do with it. If, however, the floors or landing that supports the Newel post is also carpeted, you should find there is no carpet under the Newel post, so it could be used there. If the Newel Posts were installed ON TOP of any carpet, that would almost guarantee a problem and would have to be removed before this and the Newel Post could be installed.

Q: can you combine shipping
