I just got mine - and it rattles when shaken (sounds like some small ball is inside a water tank)? Is this how it's meant to be?

mine wasnt like that...it was perfect...however, it broke down 6 months later :(

Does the steam come only from middle holes?

Yes though it does have a spray button

Does this come with a water filling plastic jug?. As I've just taken delivery of one and it does not have one in the box.
