No it does not need to be attached to anything.

How to attach it to stairs?

We did not attach it. It is so heavy it just stays in place. I have been using it for a month and it has not moved.

Could this be used with a walker?

We use this to roll large copiers (500+ poinds) unto vans. Probably could use with a walker depending on the incline
