How often on average do the batteries need to be changed?

On the 18 minute cycle, they'll last about two months. The shorter the cycle the more often you'll have to change the batteries.

Does it have the manual button to press for an extra burst of spray?

Yes, it does on the back of the spray, as well as a place to hang it from the wall.
