Q: Didn't ship with instructions. Does this fit on the knee cap or where?

It should be worn right under the knee cap. I don't understand why other people feel the need to answer in such a snarky manner. Maybe being jerks makes them feel superior.

Q: Can anyone suggest how tight to pull these straps around the knee? There is absolutely no paperwork included to guide you through using this item

Place the base strap below your knee-cap directly over the patella tendon - the strap should be centered over the tendon. Secure both ends to a comfortable snug fit. Adjust the straps to apply additional tension over the tendon; re-adjust as necessary. DO NOT over-tighten.

Q: does this use the tube inside for compression like many others

No. This product does not have the little tube as the McDavid and others do. Still, depending on what you are needing the brace for, it may work just fine. I like mine, even though I'd been using the others for several years.

Q: Will these fit an average women's knee?

Yes, definitely. Since they're covered with velcro, they can be adjusted in length quite a bit. I'm female, 5'8", 150 lbs, with what I would consider average circumference knees, and they work fine for me.
