You can choose to personalise all of this Personalised Diet Certificate by adding the slimmers name, weight achievement, consultant name, date and slimming club name or choose to personalise parts of it and write the rest of the information on yourself. If you do not tell us the WEIGHT LOSS NUMBER (1st etc), SLIMMER’S NAME, CONSULTANT NAME, DATE OR SLIMMING CLUB NAME these sections will be left blank for you to fill in. To contact us to personalise you will see a box to tick to say this is a gift, or during the checkout process you will see an option to “Add gift options” – This will bring up a box with a pre-populated gift message. Please delete the message that is there and instead leave your personalisation instructions in this box. Please save the gift option message or it won’t come through to us. Alternatively, you can go into your order once it has been placed, select problem with order and then contact seller, select “product customisation” as the contact reason, and send us a message via the messaging service.
