Q: Are these suitable for a lady with a small face?

Hello, I have a small face & they fit me.

Q: Can you wear these without glasses underneath ? Would they be uncomfortable without glasses ?

You can wear them without glasses underneath. However as they are designed to be worn over other glasses they do not have nose pieces and will not be as comfortable as a standard pair of sunglasses designed to be worn on their own.

Q: Can you do 3 in one parcel so I can save on postage?

Shipping is free, so if you buy three pairs it will still be nothing.

Q: My prescription glasses are 133mm wide. Would these still fit over?

They should fit. 133cm is 5 and a quarter inches. The over glasses fit snugly over ordinary glasses. Too much space and they would be loose and uncomfortable. I bought them for my partner and he is happy with them.
