Does this really work?

I don't have a meter to read and it doesn't block all waves, but it definitely reduces them as evidenced on the household devices

I have two separate devices, a wifi router and a modem. Does my modem also need to be covered or just the wifi router only?

Find something with copper. I tried this, and my EMF stayed the same.

Does this make the router hotter?

It is important to block wifi when you are sleeping as it affects you. So I use mine at night...before going to bed. I try to minimize any interference. It has been studied it affects sleep. I take it off when working on the computer. Otherwise it covers my modem/router
