This item was exactly what I needed.

DOT regulated, Great quality, expensive
The form is exactly what is needed for DOT Breath Alcohol Tests. The forms came nicely packaged and no damage from pressure on the carbon copies or bent corners. Nicely transfers on carbon copies when written on in a normal comfortable writing pressure. The only downfall of the product is the price. Even though it was pretty quick shipping the cost was quite inflated.
