Q: Can it hang both horizontal and vertical?

There is a removable carabiner clip but there is also an attached clip on the back as well - like the cell phone clips men attach to their belts. I think it's meant to hang vertically but could hang horizontal with the clip sliding over a belt loop. I just tried it - it still hangs on a diagonal.

Q: Will this hold an epi and an albuterol inhaler?

I think its only made to hold 2 epi pens protects them very well. And holds very secure.

Q: Can this case be put in a belt I can see the back to see if it has a loop

We do sell a ReactPaq Allergy Shot case with a fixed belt loop that you can slide over a belt, but not on Amazon at this time.
