Does the Titan plus have an integrated cup holder?

No, but I believe it's possible to purchase a Maxi Cosi cup holder separately.

Can I use the harness for 4-6 etc year old? I don’t feel comfortable just using a seatbelt for a 4 year old.

I have this car seat. A slim 4 year old can use the harness without a jacket on easily. Hard with a big jacket as the straps are really short.

Can the car seat swivel with the back tether hooked on ?

The Maci-Cosi Titan Plus is not designed to swivel. I bought it knowing this. It only points forward. It has a Centre strap that goes over the rear seat and attaches to a Centre ISO fix attachment point in the boot. This is instead of a bracket from the front of the seat to the rear seat floor pan. The seat is very sturdy and looks comfortable for our grandson 14 months and will last him for years by changing the set up as he grows. I think is was worth the extra cost for the Titan PLUS with the cooling upholstery.

Does the base swivel for ease of putting your child in the car

Not this model, no. I think you can get the swivel base models from Maxi Cosi.
